Early digital therapy applications were mostly prescriptive. They offered scripted courses, such as cognitive behavioural therapy exercises, which walked each user through the same steps, no matter their particular circumstance. Although helpful for some, this “one-size-fits-all” strategy often came across as somewhat impersonal.

Platforms nowadays are much more dynamic. AI chatbots can converse and make references to something someone said weeks before, building continuity. Apps can adapt the kind of exercises they recommend based on an individual’s answers, level of energy, or even the amount of time they appear to be willing to dedicate to a specific task. Certain systems predict when an individual is likely to be at risk of a mood dip due to changes in sleep or activity levels, and intervene with early treatment. Rather than being a passive resource, digital therapy is becoming an active companion that adjusts in real time.
The role of data in shaping experiences
This increased personalization is data-dependent. On the most fundamental level, patients complete symptom questionnaires or daily mood assessments. These self-reports continue to be the core of how apps decide what information to present. In addition to that, apps continually monitor behaviour on the platform, how long a person spends on a module, how fast they respond, or the language of their written responses.
Wearables and mobile phones introduce another layer. Sleep patterns, heart rate variability, exercise, and even location habits can be input into these systems to create a richer picture of a user’s mental condition. In hybrid models, where digital systems operate in tandem with therapists, professional observations and medical history can be included.
The hope is to construct a rich profile enabling personalized support. But the more deeply this integration proceeds, the more vulnerable the data are, and the larger the responsibility to guard them.
Do targeted therapies work?
The body of research surrounding digital mental health is encouraging but not yet full. Research has demonstrated that apps, especially conversational agents, can alleviate symptoms of depression and anxiety. Users spend more time and more regularly on interfaces that are perceived to be personalized to them, implying that personalization does enhance user experience.

The evidence, though, is diluted when it comes to long-term results. Most tests track participants for just a few months, with ongoing questions of whether benefits persist. Another headache is measurement: if personalization results in each user taking a distinct therapeutic path, it is more difficult to analyze how effective it is across the larger population. Although initial results are encouraging, the domain requires stronger, longer-duration studies to verify that personalization actually results in improved mental health outcomes, or if it primarily increases participation in the short term.
Ethical questions and privacy concerns
With increased personalization, there is an increased ethical obligation. Apps now have access to and save highly personal data, ranging from intimate journal-type entries to extensive physiological information. In 2025, reports have raised issues about platforms that store user memories forever, at times without easy ways to delete or reset them. Others have been called out over whether user data is being utilized to train algorithms outside of the immediate therapy context.
There are risks of bias as well. Where a system has been trained predominantly on data drawn from one kind of population, it can misread the experience of users from other cultural or social groups. For instance, the ways in which distress is communicated in words can differ extensively across communities, and an algorithm that ignores these variations can provide inappropriate or less good support. To preserve trust, apps need to be clear about how data is being used, provide users with control over what is being stored, and ensure diversity within the datasets that train their models.
Regulators and governments are starting to tackle these concerns. In Europe, the AI Act now imposes more stringent requirements on high-risk systems, such as those applied in healthcare. These regulations focus on transparency, accountability, and human control. In the United States, the Food and Drug Administration has started to review how generative AI sits within digital mental health, with advisory committees discussing both its advantages and risks.
Nonetheless, most wellness apps exist beyond strict regulatory systems and, therefore, uneven standards. Some platforms are sold as overall wellness devices, sidestepping the regulation that it is for official medical devices. To users, it will be confusing; what appears to be a treatment app is not necessarily subject to the same evidence or safety requirements as a regulated digital therapeutic. International coordination and clearer guidelines will be necessary to make sure personalization doesn’t get ahead of protection.

The balance between personalization and prescription
The core of the controversy is finding a balance between prescriptiveness and personalization. Bureaucratic protocols like those in cognitive behavioural therapy have decades of data behind them. They are simpler to scale, quantify, and oversee. Personalization may make treatment more relevant and exciting, but can become inconsistent and hard to prove scientifically.
In most instances, so-called personalization is in fact a fine-tuning or reordering of prescriptive content. For instance, two users can still get the same set of CBT exercises, but in varying sequences or reminders depending on their data. This hybrid model is likely to stay in the majority, combining the security of established treatment structures with the attraction of customization.
Looking ahead, by 2025, online therapy platforms will have become more adaptive, more intelligent, and more personalized. For some, that has meant mental health care that is nearer, more responsive, and more convenient. Initial studies imply real benefits, with symptom reduction for anxiety and depression in particular, but long-term efficacy remains uncertain. Meanwhile, personalization raises issues of ethics and regulation, particularly with respect to privacy and equity.

The most hopeful route is somewhere in between. Apps that pair tested therapeutic techniques with careful personalization are able to present the best of both: large-scale, evidence-backed care that also feels human and responsive. As long as developers remain open, regulators impose significant standards, and patients own their data, then digital mental health can be both personal and safe. In finding equilibrium between personalization and prescription, the future of therapy might discover its most sustainable expression.
